First have you tried other ammo and if so what are the results?
2 flavors of CCI - Brass and Aluminum, same issue on both. Run a couple of mags of Hornady TAP with no problems, but only a fraction of the ball ammo.
Second have you tore the pistol down? If so and if you have had the extractor Depressor Plunger assm out make sure it is correctly installed. I.e metal to metal and plastic to plastic or the bare metal end goes in and the plastic is what stick out out of the back of the slide until the back plate is installed.
Check the spring load bearing and see if it is excessively wore?
Torn it completely down, everything looks good. Lubed extractor, no burrs. Extractor spring and pin are straight and look fine. I used to gunsmith, so I have a good system of tear down and organizing so I get it back together right.
Have you had your mags apart and replaced the org mag spring with hvr mag springs? or put the spring in wrong?
Haven't torn them down, they feed fine in my wife's 36.
If all looks good and it continues with other ammo, load a mag to full capacity and let set for 2-4 weeks. Then try it and see what happens.
They stay fully loaded, have been for over a month.
Looks like the fwd half of the casing has been scratched? Is your chamber smooth?
Chamber is smooth, that line is from contacting the top of the barrel/chamber block when the slide closed on it. I locked the slide to the rear to take the pic.
Has the only time this has happened is at the match? If so are you limp wristing the gun? Wont make you less manly though folks will talk about you lol.
This has happened at least 1 or 2 rounds on every box of 50 I have fired through it, including at the match. I intentionally limp wristed a full mag to see if I really was causing it, I get a stovepipe when I do that. So I don't think that's the case. I've never been accused of having a limp wrist!
Have you changed hte recoil spring to a hvr/ltr wt or changed your recoil guide rod? Did you install the guide rod factory or after market correctly?

Do you have lube in the chamber/bbl ?
I dry mop the bore when I finish my cleaning, so no excess
