Do low drag followers make that big of a difference? If so where do you see the difference? In reloading, shell feed?

They can make a difference if your tube gets dirty or the transition from tube to oem tube is off. I run the Nordic ones in my nova and supernova simply because the factory follower is a flimsy plastic deal. Never saw the need to change the one in my M2 or M3k.
 
If you have plastic followers, yes, they should be changed out. A lot of shotguns come with good followers in them already, like the VM and the M2. The nose of the follower can also engage the shell latch at different locations which can result in lifter interference, so it needs to be watched.

Plastic parts, in general are good for some things, but running back and forth in steel tubes is not one of them. When they pick up a piece of sand or grit, it can end up causing some significant reliability issues.
 
If your follower has squared off edges and you add an extension to your gun, you should replace it. Many stock follower can get caught up at the junction between the extension and the original tube. Just make sure your new one has bevelled edges.
