Lost cwp

unfortunately my drivers license and cwp were lost on a trip out of town. Anyone have info on the best way to replace the cwp? The form I pulled off of the sled site for a replacement asks for a copy of the cwp?

Also need to recover my driver licenses number to do it online vs. the dmv

your suppose to notify sled within 10 days. Get your drivers license. Photo copy that and send in with either 5 or 10 dollars.
