Its not that I'm not pushing it far enough, thats as far as it will go. It just stops there.

Not that I can find. I polished the inside of the mag tube and it didn't seem to help. I did put a small bend in the lifter and that has helped a good bit, but its still not perfect.

The QLS works great.....it does only fit 2 screws, but mine has never come loose.
